Delivery & Set-Up: We cover the North Georgia Area. Delivery, service, set-up, and patient instruction are included, or you may pick up your equipment and supplies at our Jasper, GA location.
On-call Service: Special assistance is available through a 24-hour on-call service.
Discharge Assistance: We work directly with physicians and discharge planners to insure smooth transitions from institutional to home.
Emergency Preparedness: Hometown Home Health staff are trained and ready to respond to support our patients in case of natural disaster or other emergency.
Patient Instruction and Training: A trained staff of home medical equipment technicians ensures that each patient, caregivers and/or families are fully trained in the operation, care and safe use of equipment.
Patient Assessment and Evaluation: Trained staff meet with and assess the patient’s needs. We also perform an evaluation of our clients home to insure that equipment requested is adequate and well fit for the patient’s needs and abilities.
Consultations: Staff will meet, as required, with referral personnel or with the patient to solve any matter involving home medical equipment.
Reimbursement Expertise: Hometown Home Health employs a highly specialized, expert in-house billing staff. With each order, our employees work to verify eligibility and benefits as well as authorizations, and any necessary prior authorizations from your insurance company. We also employee experts in Medicare and Medicaid reimbursement.
